West High Clothes Closet
I have a supply of gently used clothing in various sizes that are available to students who may need them. If you are a student in need or know someone, just come and see me. There are no questions asked and students are welcome to what I have available. All requests are kept confidential.
Attention 9th and 10th grade students for 2009/2010
State law requires that all ninth grade students have a completed immunization series that includes:
- Series of 5 -diptheria/pertussis/tetanus
- Series of 4 Polio
- Series of 2 measles, mumps, rubella
- Series of 3 hepatitis series (*** New requirement **)
Please contact me if you have any concerns regarding your student's health and wellness. I welcome the opportunity to work with you to accommodate the needs of your child. Just a few reminders:
- If your student needs to take medication during the school day, a form needs to be completed by yourself and your physician. This authorization needs to be updated on a yearly basis.
- I do routine health assessments on students in the ninth grade and any student new to the district. This includes height, weight, eye exam, hearing screening, and blood pressure. I will contact you if there are any concerns.
- Please have your student return the emergency medical authorization cards, this is vital information if your student becomes ill or sustains an injury during the school day. Please remember to up date these numbers if they change during the course of the school year.
